Eye Drops for Cataracts? Human Trials Are Finally Here

Imagine treating a cataract the same way you treat dry eye: tilt your head back, blink twice, done. No operating room, no eye shield, no “don’t rub your eye for a week.” That is the dream researchers have been chasing for decades — and in the past few months, it officially moved from lab benches into real human trials.

Before we get carried away: no eye drop can cure cataracts today, and surgery remains the only proven fix. But for the first time, two serious drop candidates are being tested in people — and that is genuinely exciting news for anyone who has been told “you have early cataracts, let’s watch them.” Here’s the fun version of what’s happening, minus the biochemistry lecture.

First, a 30-second cataract refresher

The lens inside your eye is made of crystal-clear proteins stacked in perfect order — think of a neatly packed suitcase. As we age, those proteins start to clump and tangle, like that same suitcase after an airline has thrown it around for 60 years. Light can’t pass through cleanly anymore, and the world turns hazy, dim, and glare-y. That’s a cataract. More than half of Americans will have one by age 80.

Today’s solution is elegant but surgical: remove the cloudy lens, insert a clear artificial one. It works extremely well. But “what if drops could just... un-cloud it?” has always been the question.

Candidate #1: C-KAD — going after the metal in the mess

Researchers at the University of Utah’s John A. Moran Eye Center recently gave new life to an eye drop called C-KAD. It turns out those clumped lens proteins hoard tiny deposits of metals, and C-KAD is designed to grab and remove them — a bit like pulling the magnets out of a pile of tangled paperclips so the pile can loosen up.

In a re-analysis of a multicenter Phase 1/2 trial published in the American Journal of Ophthalmology, patients with early cataracts showed measurable improvements in lens clarity and contrast sensitivity — enough that the team, led by Dr. Randall Olson, says the drops deserve a full Phase 3 trial. His blunt summary of where we stand: there are currently no FDA-approved medicines for age-related cataracts. Surgery is the only option. C-KAD is trying to change that.

Candidate #2: lanosterol drops — the comeback kid, now enrolling humans

If “lanosterol” rings a bell, it’s because it went viral back in 2015 when animal studies suggested it could clear cloudy lenses in dogs. Then came the plot twist: other labs couldn’t always repeat the results, and the hype fizzled.

Now it’s back — properly this time. In February 2026, the first patient was enrolled in a multicenter, randomized, placebo-controlled U.S. pilot trial of ZOC2017217, a lanosterol-derived eye drop designed to discourage lens proteins from misfolding and clumping. The study kicked off at a practice in Petaluma, California, and is expected to wrap up around May 2027. Earlier exploratory data from China reported that 50–86% of treated participants reached 20/40 vision or better within a few months — numbers that need to be confirmed under stricter trial conditions, but that are hard not to smile at.

So... when can I throw away my surgery date?

Not yet — and possibly never, so please keep that surgery date. Here’s the honest scorecard:

  • These drops target early cataracts. A dense, mature cataract is a heavily tangled suitcase; no drop in trials claims to fix that.
  • Trials take years. Even if everything goes perfectly, Phase 3 testing and FDA review put any approval well into the future.
  • The science is still unsettled. Lanosterol in particular has a history of results that didn’t replicate. That’s exactly what these placebo-controlled trials are designed to sort out.
  • Online “cataract drops” are not these drops. Nothing you can buy on the internet today is the trial-grade medicine being studied. Unregulated drops can be useless at best and harmful at worst.

Meanwhile, cataract surgery remains one of the safest, most successful procedures in all of medicine — roughly 15 minutes, over 95% success, and most people notice dramatically better vision within days.

What this means if you have early cataracts in Orange County

The smartest move right now isn’t waiting for a miracle drop — it’s knowing exactly where your cataracts stand.
